Sara Crowley

Transition Coordinator for Wales, Diabetes Strategic Network

Sara Crowley is the National Diabetes Transitional Care Coordinator for NHS Wales, leading national work to improve support for adolescents and young adults moving from paediatric to adult diabetes services. She developed the award‑winning Transitional Care Diabetes National Standards and the bilingual SEREN Connect education and transitional care programme, co‑produced with young people. A Diabetes UK Clinical Champion and qualified teacher, Sara brings experience from youth offending and crime prevention to her work on co‑production, inclusive education, and easy‑read resources. Living with type 1 diabetes since childhood, she is a strong advocate for psychological support, age-appropriate care including the need for health based youth workers, disordered eating awareness, neuro-inclusion, women’s health, and eye health.
